“Errand of Mercy” has one of the bloopers that they just shrugged, heck with it:  John Colicos’ lieutenant is caught on camera standing, waiting for his cue to step out and go striding down the corridor to deliver his report.

No time to re-shoot, and as was famously said, it was just a TV show, for cryin’ out loud.
